Departmental approval is required before a student may register for an online section of MATH 1111.  Please read the following information carefully.  By completing this form and submitting your request to the Math Department, you are agreeing to all terms and conditions on this page.  If you are approved, you must withdraw from any other section of MATH 1111 before we can enroll you in the online section.

MATH 1111 is taught every semester on-campus and in an online format during the Fall semester.  The course content for MATH 1111 is the same for the on-campus and online sections.  However, in the online course, all communication between student and instructor occurs via technology, primarily through e-mail and MyMathLab/CourseCompass (a web-based course management system).  To succeed in this course, you need to be motivated, with the self-discipline to take responsibility for your own learning.  However, many motivated students still struggle with the quantitative literacy required to succeed in this course.  Therefore, you also need the ability to reason and think critically about quantitative situations.  This online course is NOT self-paced.  There is a strict schedule for the keeping up with the lessons, completing regular assignments, taking online quizzes, and communicating with the rest of the class.

To register for the MATH 1111 online course, you must complete the following form.  The information will automatically be emailed to the instructor.   THE APPROVAL PROCESS MAY INDICATE FACTORS THAT SUGGEST YOU ARE NOT LIKELY TO SUCCEED.  IF SO, YOU ARE DENIED APPROVAL.  APPROVAL DOES NOT GUARANTEE SUCCESS IN THE COURSE.

Before filling out the following form, please click here and read all the information about the course including the syllabus.  One of the questions below is from the syllabus.
